- Eligibility Requirements for NHTD Program
- 18 years to 64 years with a traumatic brain injury
- eligible for the nursing home level of care
- have active Medicaid or qualify for Medicaid coverage for community-based long-term care
- the individual must also be able to live safely in the community with the services and supports offered through Medicaid, the waiver program and other community resources
- diagnosed with a Traumatic Brain Injury (TBI)
- living in New York State
